Family Physicians
The physicians provide the full spectrum of primary, holistic client care with the support of an inter-disciplinary team of health care providers and support services.
Services include: chronic disease management, diagnosing and treating acute illness and minor injuries, preventive care, including
routine checkups, health-risk assessments, immunization and screening tests, pre-natal care, and referrals to specialists as required.
Nurse Practitioner
The Nurse Practitioner provides same-day appointment services and works in collaboration with family physicians and health care providers to co-manage clients of the health centre and Canwood clinic.
Services include: Chronic disease management, health promotion and protection, disease and injury prevention, provision of supportive, instructive, treatment and urgent care services, and referrals to other health care professionals
The Co-operative Health Centre/Community Clinic welcomes new clients. To ensure access to service to all of our clients, we have a scheduling algorithm in place to determine which physicians currently accept new clients.
